Effective immediately, there is no longer a need to complete the "paper" INFLUENZA & PNEUMONIA VACCINATION worksheet (attachment A) during your annual survey.

The questions have been revised and added to the Long Term Care Questionnaire that you are required to complete annually by April 15th. Attached is a copy of the revised questions that you can expect to see on the Long Term Care Questionnaire this year, along with the directions for those questions.
