As per DOH Message Board dated 9/10/2014:

All facilities are required to prohibit discrimination against persons receiving services in a facility or program and employees. Below is an updated Nondiscrimination Notice for facilities and programs operated in accordance with the regulations of the Pennsylvania Department of Health. This Nondiscrimination Notice replaces information that had been posted from years past. Direction is provided for all inquiries and complaints regarding civil rights discrimination as indicated by facility type. In addition, information is provided for employment discrimination complaints initiated by facility/provider staff.
