Registered Nurse Assessment Coordinator (RNAC) – Full-Time

Fairmount Homes, a faith-based Continuing Care Retirement Community, is seeking an experienced RN to coordinate MDS assessments, admissions, and Medicare documentation. This role partners with the interdisciplinary team to ensure regulatory compliance, accurate care planning, and quality outcomes.

Qualifications: PA RN license, 2+ years long-term care experience, working knowledge of MDS/Medicare PPS/CMI, strong organizational and communication skills.
