Description

Manages and assumes responsibility of all aspects of the delivery of nursing services by appropriately using clinical and administrative knowledge base through responsibility and accountability.

Duties to Include (but not limited to): Maintains ongoing survey readiness (policies, documentation, staffing, competencies, QA monitoring). Coordinates nursing response during state/CMS surveys; ensures staff are prepared to answer questions and demonstrate processes. Leads/assists with plans of correction (POCs), allegations of non-compliance, and sustained compliance monitoring after survey. Ensures nursing policies/procedures align with current federal/state rules and evidence-based practice. Available 24/7 on call. On call responsibilities shared with Evening Nursing Supervisor. Able to work a shift if unable to cover it to maintain state staffing ratios. Responsible for coordination of staffing schedule via OnShift scheduling software. Leads daily clinical rounds to review resident physical, mental and emotional status and implements required nursing intervention.

Required Attributes:

  • Education: A minimum of an Associate’s Degree in Nursing is required. Bachelor of Nursing Degree or higher is preferred.
  • Licensure: Current (active) unencumbered Pennsylvania Registered Nurse’s License is required.
  • Experience: Minimum of 2-years’ experience in long term care facility or a supervisory capacity in a hospital with focus on rehabilitation. 3-5 years’ experience in long-term care facility with skilled beds preferred. Intermediate knowledge of MDS and care plan process is required.
  • Physical: Must be able to perform the duties of this job with or without reasonable accommodation, including but not limited to: Standing for up to 7 hours of 8-hours shift. Push, pull, tug, twist, bend, stoop and sit, on a regular basis. Occasionally lift up to 50 – 100 lbs. with assistance.
