The 2013 Long Term Care Questionnaire conducted by the Department of Health, Bureau of Health Statistics and Research will be mailed out to the facilities by this Friday, February 28, 2014. Facilities should look for this survey to arrive in the United States Postal Service the week of March 3, 2014. The due date for completion of the Questionnaire is April 15, 2014.

Failure to complete and submit the Long Term Care Questionnaire could result in deficient practice.
