The 2014 Long Term Care Questionnaire conducted by the Department of Health, Bureau of Health Statistics and Research, was mailed out to the facilities on February 27, 2015.

Reminder: the due date for completion of the Questionnaire is April 15, 2015.

Failure to complete and submit the Long Term Care Questionnaire could result in deficient practice.

The attachment contains training slides that were developed based on the changes that were made last year to the Immunization sections of the Long Term Care Survey (Sections I & J) and instruction for completion.
