Full-time / Exempt / Pay based on experience

Reporting to the Administrator, the MDS Coordinator serves as the Resident Assessment Coordinator and Infection Control Preventionist at Chambers Pointe Health Care Center, overseeing and ensuring the accurate completion of the Resident Assessment Process while managing on-call responsibilities with a commitment to excellence and accountability.

Qualifications:

• Licensed by the Commonwealth of PA as a Registered Nurse (Required)
• Experience in Long-Term Care Nursing (Preferred)
• Infection Control Certification or the ability to obtain certification (Required)
• Experience with software platforms such as Microsoft Suite (Office, Excel, Word) (Required)
• Strong interpersonal skills with the ability to build relationships at all levels (Required)
• Strong written and verbal communication skills (Required)
• Excellent organizational and time management skills (Required)
• Excellent attention to detail (Required)
• Proactive problem-solving mindset (Required)
• Collaborative team player with a positive and professional demeanor (Required)

Shift:

• Day Shift: 7:30am-4PM, Salaried
