Description

A Wound Care Nurse (RN) provides nursing wound care to residents as prescribed by a physician and in accordance with nursing practices standards.  Reports to Director of Nursing.

General Purpose is to monitor and manage the wounds within the facility.  Provide guidance to the nursing units on the importance of prevention of wounds as well as care of wounds when they exist.  In addition, oversee and guide the facility so that facility Wound Management remains within the guidelines of State and Federal Regulations.

Under the supervision of the DON and following State and Federal Regulations, individual is responsible for the teaching, monitoring and implementation of wound protocols.  Must be proficient in appropriate wound protocols, treatment modalities, comfort measures, documentation, and terminology of wounds. The incumbent is directly responsible for initiating and monitoring wound programs for individual residents. Responsible for knowing State and Federal Regulations, especially as they relate to wound care.  Individual needs to develop Wound programs.  Also, orient new licensed staff to their specific role in collaboration with this position.

Specific responsibilities of the position include, but are not limited to, the following:

  • Round daily on the nursing units to monitor compliance with wound management interventions, protocols, policy and procedure;
  • Collect and report data as it relates to Wound Management;
  • Work in conjunction with the Quality Assurance and other committees as it relates to Wound Management;
  • Rounds with physicians and other professionals as it relates to wounds
  • Provide frequent education to the nursing staff to promote compliance with Wound Management and related policies and procedure;
  • Ensures physician orders are carried out accurately and timely
  • Ensures changes in resident condition is reported promptly to immediate supervisor
  • Collects and reports on appropriate data on wounds – hospital acquired, in house acquired, other
  • Lead routine wound rounds on the units and document the same in accordance with requirements;
  • Coordinate the completion of routine skin assessments of residents; and able to summarize assessments and make sound nursing diagnosis for the optimal outcomes for resident
  • Provide communication with physicians regarding wound care of residents and suggestions for interventions to promote healing;
  • Liaison with the local wound care clinics;
  • Able to respond and act accordingly to emergency situations
  • Maintain policy and procedures, wound protocols for the facility in accordance with standards of care;
  • Round on the nursing units to act as an additional resource person as needed
  • Attend meetings as related to Wound Management and other aspects of the position;
  • Prepare reports to the DON and/or Administrator;
  • Assist with counseling of nursing staff if requested;
  • Complete frequent documentation such as wound flow sheets, nurse’s notes, care plans, etc;
  • Mentor new licensed staff during initial orientation period, providing coaching and discipline when necessary
  • Familiarize self with State and Federal Regulations to be a resource person to the staff on the units
  • Other duties as assigned by the DON/designee
  • Have ability to manage stress tolerance, pressure and anxiety
  • Although unlikely, local and out-of-town travel may be required for training purposes
