Description

FT Weekend RN

  • Supervise nursing activities of his/her nursing assignment; organize and assign responsibilities based upon resident population, staff qualifications, and physical structure, supervise nursing aides, assist nurse aides in providing care and service to residents, evaluate nurse aides based upon their job requirements and the expectation of their residents.
  • Make rounds to observe and evaluate physical, emotional, and social needs of residents; prepare and administer medications; give treatments; consult with physicians, administer first aid during emergencies.
  • Assist in training of personnel, problem solving, maintain records (including electronic records) reflecting residents' condition, medication, and treatments.
  • Communicate with Director of Nursing and all members of the interdisciplinary team.
  • Promote highest level of independence with all residents, encouraging them to participate in their care and allowing them decision making responsibility.
  • Complete all required documentation. Administer care and service while meeting all requirements of regulatory oversight. Maintain HIPAA compliance.

 

Reports to: Director of Nursing and Assistant Director of Nursing

 

Position-Specific Responsibilities & Duties; Primary Essential Duties:

  1. Provide care to residents living in the skilled nursing care in a Primary Care model.  Administer medications in accordance with professional standards and manufactures recommendations. Provide treatments as ordered by the physician, assess residents for change in status, and document appropriately on all.
  2. Provide leadership in assignment, giving direction to the CNA’s, ensuring that each resident’s needs are being met and participating in their ongoing success and performance improvement process.
  3. Assess each resident for changes, document and report any changes to attending physician and primary Power of Attorney (POA).
  4. Provide all required documentation for residents, including testing results, lab work completion, and medication administration. Sign off of physician orders correctly and completely, admission and discharge paperwork as well as routine vital signs, skin assessments, behavioral and pain records.
  5. Participate in development of resident individual care planning process.  Including assessment of needs, recognition of changes and updating of forms timely.
  6. Identify opportunities to improve care delivery, including process enhancements or policy alteration.
  7. Evaluate effectiveness of CNA’s.  Assist in completion and administration of Introductory and Annual reviews.  Utilize the corrective action/disciplinary process to assist CNA’s to recognize opportunities to improve their care delivery and meet the expectation of their job.
  8. Effectively communicate with the interdisciplinary team to ensure that changes and new information is shared with all involved.
  9. Provide and document patient teaching.  Identify learning needs related to discharge plans, new diagnosis, anticipated testing and any new medications.
  10. Attend all required in-service training to meet regulatory requirements.  Maintain excellent nursing skills.  Participate in additional in-services through our own staff development office and outside resources.  Attend monthly Team Leader meetings.
  11. Provide residents and families with excellent customer service, striving to exceed their expectations.  Maintain highest level of integrity and adhere to corporate compliance and HIPAA policy.

 

Position Qualifications:

  1. Registered Nurse or Licensed Practical Nurse by Board of Examiners in PA, in good standing
  2. Completed pharmacology course.
  3. Current CPR certification
