Participants must attend the workshop in its entirety, complete the post-test and submit to the speaker at the conclusion of the workshop. Participants will receive their CNE certificate for 5.75 contact hours upon attendance at the event and successful completion of post-test (participant must score 80% or higher).

Avoidable hospital admissions are a key patient safety and quality management concern. One in five Medicare enrollees is re-admitted to the hospital within 30 days of hospital discharge and up to 75% of these re-admissions are preventable. Sadly, the re-admission rate for patients discharged to skilled nursing homes is even higher with 25% re-admitted within 30 days! Governmental and third-party payors seek in-network providers with solid nursing skills, care protocols, and resident outcomes.

This interactive workshop will focus on the "how to" of performing a thorough physical assessment, documentation, and early intervention strategies for the top 3 re-admission diagnoses: Congestive Heart Failure; Chronic Obstructive Pulmonary Disease and Pneumonia. Attendees will learn a number of strategies that show promise in effectively reducing re-admissions.

Participant will be able to:
1. Perform nursing assessment for CHF, COPD and pneumonia to detect abnormal findings
2. Document physical assessment findings and physician notification
3. Identify interventions that may prevent exacerbation of symptoms

This workshop is geared toward Directors of Nursing, Assistant Directors of Nursing, Unit Managers, RN Supervisors, and staff nurses working in long-term care. This course is limited to 40 participants and will be offered on a first come/first serve basis.

Rebecca Flack (BSN, MSN, CRNP, DNP)
Rebecca has been a nurse for over 30 and a DON in long-term care for over 20 years. She obtained a BSN, MSN and CRNP from the University of Pittsburgh in 1995 and a DNP from Waynesburg University in 2010. She has been adjunct faculty at Waynesburg University, Carlow University and Westmoreland County Community College. She was Manager, Clinical Quality Assurance for Focus Care 2011-2016. CRNP experience includes community health nursing. Experience includes a CMS Contracted Reviewer for various insurance plans throughout the eastern United States in 2015.
